Breaker Failure.
Not my field but I think some aspect of the circuit breaker failed and some back EMF bridged the breaker side of the switch. If you take a look there is a piece that rapidly breaks the circuit first before the arm opens, the arc flash starts there.

Arc Flash Demo.
This is a lab test, showing arch flash. The radiant heat must be enormous, the blobs of metal are noteworthy as well.

Electric Arc Furnace.
An electric ARC furnace, caution its loud. The scale of this is significant.
